Application Forms
NOTE:  As of January 1, 2016 prices have increased for Recertifications/Renewals, Pesticide Exams, and Reciprocity Applications.

The following applications are not accepted via fax or email, as we require your original signature in ink:

Dispenser Application - This form is for Dispenser renewal, reciprocity, and replacement certificates,  Also if you currently hold a full, unrestricted Pesticide Applicator certificate and would like a dispenser certificate you can submit this completed form to our office for that.

Applicator Recertification - This form needs to be submitted to have your Applicator certificate(s) recertified. 

Applicator Reciprocity - If you have a valid certificate in another province and would like to have an Alberta certificate issued, you will need to submit this completed application to our office.

Applicator Replacement - If you require another copy of your certificate, you must submit this form to our office.

Special Certificate Grading - If you are submitting Lesson 505, 801 or 804 for grading, this form must be included with the Lesson or Lesson cannot be graded.

Name Change - If you have changed your name and would like us to update it on our system, you must submit this form along with the required documentation.  If you require a new certificate to be printed please submit the Applicator Replacement application as well.
Home Study Material Order Form
Home Study Material Order Form - This form provides costs and can either be mailed or faxed, or you can phone 1 866 853 8646 and place the order over the phone.  Acceptable forms of payment are: cash, cheque, money order, visa and MasterCard.

  • Complete course packages consist of Core Lesson (common to all certification classes) and a series of lessons that differ depending on the certification class selected.
  • Individual lessons can be ordered separately to add other certification classes.

Other Application Fees:  This lists fees for various services. 
Credit Chart - This table is useful when you are verifying that you have earned enough credits in each concept area to recertify.
Training Check List for Pesticide Applicator Assistants
The Training Checklists must be completed with supervising certified applicators: Prior to any pesticide application; whenever the pesticide, application equipment or use location changes; and at least yearly afterwards.
