Dental Care |
- Basic services covered at 100%.
- Major/Extensive services covered at 80%.
- Combined maximum of $1,500 per person per year for Basic & Major/Extensive services.
- Orthodontics covered at 50% to a maximum of $2,500 per lifetime for dependent children less than 19 years of age.
- Basis of payment is the 2007 Alberta Blue Cross Usual & Customary Dental Schedule of Fees.
- Coverage for Part‐Time employees is equal to one‐half of coverage indicated above:
- 50% Basic services
- 40% Major/Extensive services
- $750 combined Basic and Major/Extensive benefit year maximum
- 40% Orthodontic Services to a lifetime maximum of $1250 per dependent child

Health Spending Account |
- You may be eligible to receive an annual Health Spending Account (HSA) allocation based on your employee classification and dependent status.
- You may use your HSA to pay for eligible health, dental or vision expenses not covered by the group benefit plan.
- The HSA plan year runs August 1 to July 31 each year.
- HSA dollars are allocated monthly.

Administration Employees:
- Death benefit of 3 x annual earnings (to a maximum of $200,000).
Faculty Employees:
- Death benefit of $75,000.
AUPE Employees:
- Death benefit of 2.5 x annual earnings (to a maximum of $100,000).
Your benefit reduces by 50% on your 65th birthday, and terminates at age 70.
Optional:
- Employee & Spouse – units of $10,000 to $250,000 maximum for each covered person (25 units).
Your and/or your spouse’s benefit terminates once you reach age 65. |

Voluntary:
- Employee only – units of $25,000 to $250,000 (10 units).
- Employee & Family Plan – units of $25,000 to $250,000 (10 units); varying percentages of employee coverage for spouse & dependent children.

Administration Employees:
- Taxable benefit equal to 75% of monthly earnings up to a maximum monthly benefit of $7,500.
- Commences after qualifying period of 130 working days or 6 months (whichever is shorter) of continuous disability.
Faculty Employees:
- Non‐taxable benefit equal to 70% of monthly earnings up to a maximum monthly benefit of $3,000.
- Commences after qualifying period of 130 working days or 6 months (whichever is shorter) of continuous disability.
AUPE Employees:
- Taxable benefit equal to 75% of monthly earnings up to a maximum monthly benefit of $3,000.
- Commences after qualifying period of 80 working days or 4 months (whichever is shorter) of continuous disability.
