Pesticide Certification Program - Forms Fees & Exams

In this section, you will find application forms for:

These forms can be filled out and returned by mail or scanned (PDF) and emailed to the pesticide office. 
Other forms available are Alberta Environment.
Application forms - All require your signature
The following applications are accepted via email (PDF format only), signatures are required on the forms.  Email to pesticides@lakelandcollege.ca
Dispenser Application form (pdf)- This form is for dispenser renewal, reciprocity, and replacement certificates,  Also if you currently hold a full, unrestricted Pesticide Applicator certificate and would like a dispenser certificate you can submit this completed form to our office for the additional certification.

Applicator Recertification form (pdf)- This form needs to be submitted to have your Applicator certificate(s) recertified. 

Applicator Reciprocity form (pdf) - If you have a valid certificate in another province and would like to have an Alberta certificate issued, you will need to submit this completed application to our office.

Applicator Replacement form (pdf)- If you require another copy of your certificate, you must submit this form to our office.

Special Certificate Grading form (pdf) - If you are submitting Lesson 505, 801 or 804 for grading, this form must be included with the lesson. If it isn't included, the lesson can't be graded.

Name Change form (pdf) - If you have changed your name and would like us to update it on our system, you must submit this form along with the required documentation.  If you require a new certificate printed, please  also submit the Applicator Replacement form.

  • Complete course packages include the Core Lesson (common to all certification classes) and a series of lessons that differ depending on the certification class.
  • Individual lessons can be ordered separately to add other certification classes to your qualifications.
How to order

 2.       Return the completed order form to the pesticide office

 3.       Pay the Fee

  • Cash (in person only)
  • Cheque or money order (mail or in-person)
  • Visa or MasterCard
Exams and proctored exam locations
What you need to know:
  • you book your exam by contacting the pesticides office in Vermilion at 1 866 853 8646 or fill out Exam Request form ( pdf) then email to the Pesticide office, after submitting call the office at 1 866 853 8646 or 780 853 8646 to book a date and time.
  • exams vary in length from 1.5 - 4.5 hours in length
  • all are closed book
  • once you pay the exam fee, you ​are sent an email with instructions on how to write the online exam
  • exams are available in an electronic format in a virtual setting. Due to the virtual setting of the exam the NAFTA Core and 1 Technical exam may be combined, please note they will still be graded as separate exams and you must pass the NAFTA Core to pass the Technical
  • are specific to the class(es) of applicator certification
  • you may challenge the exam without doing the homestudy course. Note that the lessons are specifically designed to help you pass the certification exam
  • 70%, or better, is the pass mark
  • you may apply for a second rewrite if you get less than the passing grade

Alberta Environment forms & resources
All of Alberta Environment's pesticide applicator related forms are available on the pesticide management application forms page. Those forms include:
  • Pesticides - Application forms for Service Registration- applying pesticides as a service, on rights-of-ways, public land or for forest management.
  • Pesticides - Application forms for ​Workshop/Conference Request for Credit - course or event coordinators use to apply for pesticide applicator credits at a workshop/conference or a training event
 Authorized assistant training check list:  The Training Checklist must be completed with supervising certified applicators: Prior to any pesticide application; whenever the pesticide, application equipment or use location changes; and at least yearly afterwards.  The training checklist is also available in the Environmental Code of Practice for Pesticides.
