Before you get too far into the process, please contact the nominee and tell them you are nominating them for this award. You'll need their resume or CV, so you'll need their cooperation and assistance.

Supporting documents

  1. Nomination letter - You may use the spaces in the form, or write a nomination letter answering the 4 questions. Please be brief, about 100 words per question.
  2. Supporting letters - Please submit at least one letter of support. The limit is 3. Each letter should only be a page long and include the details of a specific achievement.
  3. Nominee's resume or CV - Please submit one or the other of these documents.

Distinguished Alumni Awards recognize graduates who have demonstrated professional excellence and leadership, and who have been an inspiration to fellow alumni, current students and the community.
 
In addition to this web form, your nomination must include the following:
  • Confirmation you have told the nominee about the nomination
  • A nomination letter, or filling out questions below
  • Support letters
  • The nominee's resume or curriculum vitae (CV)
